Translated Abstract
Objectives:
Through observe the self-care agency and psychology changes after transitional care, in acute myocardial infarction patients after PCI, to discuss its effects and provide ideas and basis for future nursing.
Methods:
Our observed objects were acute myocardial infarction patients accepted PCI. This study used quantitative research methods, study objects were divided into experimental and control groups randomly. Experimental group received routine care, discharge guidance and transitional care intervention, control group received routine care and discharge guidance. Transitional care interventions included home visits regularly, focus on learning to hospital, telephone follow-up, and the establishment of micro-letters and QQ group, explaining disease-related knowledge and psychological counseling. Investigated the self-care agency, anxiety, depression and readmission rate survey six months pre-and post-intervention to evaluate the effect of transitional care. The main research tools were general information questionnaire, clinical data of questionnaire, self-care agency scale (ESCA), self-rating anxiety scale (SAS), self-rating depression scale (SDS), readmission registration form.
After the questionnaires, data entry EpiData3.0 establish a database for analysis using SPSS 17.0. Statistics is described by frequency, proportion, mean, standard deviation, statistical inference data using t test, ?2 test.
Results:
1. The general situation and clinical data of research object: a total of 82 patients were included, 40 cases in experimental group, (male 28 cases, female 12 cases); 42 cases of control group, (male 27 cases, female 15 cases); the two groups in gender, age, culture degree, disease type, the result of coronary angiography, the difference was not statistically significance (P>0.05).
2. There was no significant difference(P>0.05)of the total self-care agency score and each dimension score between the two groups before the intervention. After transitional care six months later, the experimental group scores were significantly higher than control group(P<0.05): with self-care agency total score (117.73±8.92 and 103.62±7.11), self-care skills score (32.23±4.97 and 20.90±4.34),self-responsibility score (20.95±2.05 and 14.62±4.29),self-concept(28.58±2.09 and 18.24±4.27),health knowledge levels (50.43±5.89 and 40.17±6.06),respectively.
3. Before the intervention, control group with anxiety score (45.86±7.25)and the score of experimental group (45.73±9.06)had no significant difference(P>0.05), and with statistically difference (P<0.05) after the implementation of transitional care six months later. SAS score was lower in experimental group (39.23±6.31) than in control group score (44.67±6.10).
4. Before the intervention, control group with depression score (45.82±7.03)and the score of experimental group (47.88±7.53)had no significant difference(P>0.05), and with statistically difference (P<0.05) after the implementation of transitional care six months later. SDS score was lower in experimental group (41.25±7.95) than in control group score (47.02±5.73).
5. Patients were readmitted within 6 months of the case: the readmitted cases within 6 months was significantly lower in the experimental group (1/40 cases) than the control group (7/42 cases),with statistically significant (P<0.05).
Conclusions:
The self-care agency was improved, the anxiety, depression was eased, readmission rate also decreased significantly after implementation of transitional care of acute myocardial infarction patients after PCI.
