Effects of orthotic insoles on adults with flexible flatfoot under different walking conditions. PubMed Scopus

Abstract :

[Purpose] This study was to evaluate the effects of orthotics on adults with flexible flatfoot when wearing orthotic insoles while walking on horizontal ground, walking up and down stairs and to determine if flexible flatfoot needs treatment. [Subjects and Methods] Fifteen college students with flexible flatfoot and fifteen college students with normal feet were recruited. First, load rate and contact area were measured by RSscan force plate when the subjects were walking on horizontal ground, walking up and down 10 cm and 20 cm stairs. Then the subjects with flexible flatfoot were instructed to wear orthotic insoles for 3 months, and plantar pressure was measured again. Finally, the data were subjected to repeated measures ANOVA. [Results] After treatment for 3 months, the plantar pressure of flatfoot was significantly improved. In addition, the data of the subjects with normal feet and flatfoot were significantly influenced by walking down 10 cm or 20 cm stairs. [Conclusion] Orthotic insoles could significantly improve the plantar pressure of flatfoot. Additionally, the arches of subjects with normal feet and flatfoot can be significantly deformed when walking down stairs. Therefore, it is essential for subjects with flexible flatfoot to wear orthotic insoles to avoid needless injury.

Abstract :

Serotonin (5-hydroxytryptamine, 5-HT) is an important neurotransmitter that modulates N-methyl-D-aspartate (NMDA) receptor activity by binding to several different 5-HT receptor subtypes. In the present study, we used whole-cell patch-clamp recordings in transverse slice preparations to test the role of 5-HT receptors in modulating the NMDA receptor-mediated miniature excitatory postsynaptic currents (mEPSCs) in layer II/III pyramidal neurons of the rat visual cortex. We found that the NMDA receptor-mediated component of mEPSCs could be potentiated by exogenously applied 5-HT. Similar results were obtained by exogenously applied 5-CT or 8-OH-DPAT (the 5-HT1A and 5-HT7 receptor agonist). A specific antagonist for the 5-HT7 receptor, SB-269970, completely blocked the increase in NMDA receptor-mediated component of mEPSCs by 5-CT or 8-OH-DPAT. Moreover, the selective 5-HT1A receptor antagonist, WAY-100135, displayed no influence on the enhancement in NMDA receptor-mediated component of mEPSCs by 5-CT or 8-OH-DPAT. These results indicated that the increase in NMDA receptor-mediated component of mEPSCs by 5-HT in layer II/III pyramidal neurons of the young rat visual cortex requires activation of 5-HT7 receptors, but not 5-HT1A receptors. These observations might be clinically relevant to schizophrenia and Alzheimer's disease (AD), where enhancing NMDA receptor-mediated neurotransmission is considered to be a promising strategy for treatment of these diseases.

人参皂苷Rg1对AD大鼠脑片模型Bcl-2表达的影响

Abstract :

目的 观察人参皂苷Rg1对冈田酸(okadaic acid,OA)诱导的AD大鼠脑片模型Bcl-2表达的影响.方法 将5周龄SD大鼠脑片随机分为对照组、模型组和小、中、大剂量人参皂苷Rg1组,每组10张脑片.各组脑片首先至于人工脑脊液中不做任何处理恢复1h;然后Rg1小、中、大剂量组分别加入Rg1致使其终浓度为60、120、240 μmol/L,培养至2h;然后将OA加入模型组和人参皂苷R1各组中,致使其终浓度分别为1μmol/L,作用3h,空白对照组不加任何处理.干预结束后,采用Westornblot技术检测各组大鼠脑片Bcl-2蛋白表达水平.结果 与对照组比较,模型组大鼠脑片Bcl-2的表达差异有统计学意义(P<0.01);与模型组比较,3个剂量人参皂苷Rg1组Bcl-2表达差异有统计学意义(P<0.01),其中以大剂量人参皂苷Rg1组Bcl-2表达水平最高.结论 人参皂苷Rg1可能通过上调Bcl-2基因表达途径从而抑制神经元凋亡,对AD发挥神经保护作用.

Abstract :

Background and Purpose-Mechanisms, acute management, and outcomes for patients who experience intracerebral hemorrhage may differ from patients with ischemic stroke. Studies of very early rehabilitation have been mainly undertaken in patients with ischemic stroke, and it is unknown if benefits apply to those with intracerebral hemorrhage. We hypothesized that early rehabilitation, within 48 hours of stroke, would improve survival and functional outcomes in patients with intracerebral hemorrhage. Methods-This was a multicenter, randomized controlled study, with blinded assessment of outcome at 3 and 6 months. Eligible patients were randomized to receive standard care or standard care plus early rehabilitation. Primary outcome includes survival. Secondary outcomes includes health-related quality of life using the 36-item Short Form Questionnaire, function measured with the modified Barthel Index, and anxiety measured with the Zung Self-Rated Anxiety Scale. Results-Two hundred forty-three of 326 patients were randomized (mean age, 59 years; 56% men). At 6 months, patients receiving standard care were more likely to have died (adjusted hazard ratio, 4.44; 95% confidence interval [CI], 1.24-15.87); for morbidity outcomes, a 6-point difference in the Physical Component Summary score of the 36-item Short Form Questionnaire (95% CI, 4.2-8.7), a 7-point difference for the Mental Component Summary score (95% CI, 4.5-9.5), a 13-point difference in Modified Barthel Index scores (95% CI, 6.8-18.3), and a 6-point difference in Self-Rating Anxiety Scale scores (95% CI, 4.4-8.3) was reported in favor of the intervention groups. Conclusions-For the first time, we have shown that commencing rehabilitation within 48 hours of intracerebral hemorrhage improves survival and functional outcomes at 6 months after stroke in hospitalized patients in China.

Health-related quality of life and long-term care needs among elderly individuals living alone: a cross-sectional study in rural areas of Shaanxi Province, China SSCI SCIE PubMed Scopus

Background: The number of elderly individuals living alone is rising, especially in rural areas of China, and their health-related quality of life (HRQoL) is an increasing public health concern. However, little is known about factors that influence HRQoL and the need for long-term care services. The aim of the study was to identify these factors and the long-term care requirements of persons aged 60 and older living alone in rural areas of Shaanxi Province, China. Methods: The study included 424 older subjects, selected by stratified random sampling. Logistic regression adjusted for age was conducted to analyze factors influencing HRQoL and the need for long-term care services. Pearson correlative analysis was conducted to assess the correlation between HRQoL score and long-term care needs. Results: HRQoL among elderly subjects living alone declined with age in both males and females. The main diseases influencing HRQoL among the elderly were hypertension, cardiac disease, chronic bronchitis, neurological disease and cancer. Cataract disease was the most important factor related to HRQoL. This was followed by long-term care needs, living conditions, economic status, Cardiovascular disease, osteoporosis and age. Factors affecting long-term care needs were economic status, education level, alcohol intake, living conditions, general health and age. HRQoL and long-term care needs among this elderly population were significantly correlated (r=-0.204, p<0.01). Conclusions: For elderly persons living alone, factors such as chronic disease, lower income level and living in a rural area may limit their ability to form social relationships. Reducing the level of loneliness, with better care and support, may be helpful in improving their HRQoL. There is a need for an overall improvement in the planning, provision and financing of long-term care and psychogeriatric services for elderly individuals living alone in China.

N-methyl-D-aspartate receptors (NMDA-Rs) activation has been implicated in various forms of synaptic plasticity depending on the receptor subtypes involved. However, the contribution of NR2A and NR2B subunits in glycine-induced long-term potentiation (LTP) of miniature excitatory postsynaptic currents (mEPSCs) in layer II/III pyramidal neurons of the rat visual cortex remains unclear. The present study used whole-cell patch-clamp recordings in vitro to investigate the role of NR2A-containing and NR2B-containing NMDA-Rs in glycine-induced LTP in visual cortical slices from 13-15 day old rats. We found that glycine-induced LTP of mEPSCs was readily induced in layer II/III pyramidal neurons of the rat visual cortex with glycine. D-APV, a selective NMDA-R antagonist, blocked the glycine-induced LTP. Moreover, the selective NR2B-containing NMDA-R antagonists (Ro 25-6981) displayed no influence on the glycine-induced LTP. However, Zn2+, a voltage-independent NR2A-containing NMDA-R antagonist, prevented glycine-induced LTP. These results suggest that the glycine-induced LTP in layer II/III pyramidal neurons of the rat visual cortex is NMDA-R-dependent and requires NR2A-containing NMDA-Rs, not NR2B-containing NMDA-Rs.

Abstract :

Ginsenoside Rg1 inhibits oxidation, aging and cell apoptosis, and improves cognitive function. In this study, we pretreated rat brain tissue sections with ginsenoside Rg1, and established brain slice models of Alzheimer's disease induced by okadaic acid. The results revealed that ginsenoside Rg1 pretreatment suppressed the increase in phosphorylated Tau protein expression induced by incubation with okadaic acid, and reduced brain-derived neurotrophic factor expression. These results suggest that ginsenoside Rg1 upregulates brain-derived neurotrophic factor expression and inhibits Tau protein phosphorylation in brain slices from a rat model of Alzheimer's disease.

Activation of N-methyl-D-aspartate receptors (NMDARs) has been implicated in various forms of synaptic plasticity depending on the receptor subtypes involved. However, the contribution of NR2A and NR2B subunits in the induction of long-term depression (LTD) of excitatory postsynaptic currents (EPSCs) in layer II/III pyramidal neurons of the young rat visual cortex remains unclear. The present study used whole-cell patch-clamp recordings in vitro to investigate the role of NR2A- and NR2B-containing NMDARs in the induction of LTD in visual cortical slices from 12- to 15-day old rats. We found that LTD was readily induced in layer II/III pyramidal neurons of the rat visual cortex with 10-mm 1-Hz stimulation paired with postsynaptic depolarization. D-APV, a selective NMDAR antagonist, blocked the induction of LTD. Moreover, the selective NR2B-containing NMDAR antagonists (Ro 25-6981 and ifenprodil) also prevented the induction of LTD. However, Zn(2+), a voltage-independent NR2A-containing NMDAR antagonist, displayed no influence on the induction of LTD. These results suggest that the induction of LTD in layer II/III pyramidal neurons of the young rat visual cortex is NMDAR-dependent and requires NR2B-containing NMDARs, not NR2A-containing NMDARs.

Objective: To observe the effect of a Chinese medicine compound, Naoerkang ((sic), NEK), on amyloid-beta peptide (1-42; A beta (1-42)) and matrix metalloproteinase-9 (MMP-9) expressions in the hippocampus of Alzheimer's disease (AD) model rats. Methods: A total of 48 male Sprague Dawley (SD) rats were randomly divided into normal control, untreated, and piracetam groups, and low-dose, medium-dose, and high-dose NEK groups, with 8 rats in each group. The 5-mu L aggregated A beta (1-42) (2 mu g/mu L) were injected into both CA1 areas of the hippocampus in the rats to establish an AD model, whereas the normal control was treated with the same dose of normal saline. The rats in the NEK groups were treated with a high, medium, or low dose of NEK [60 g/(kg.d), 30 g/(kg.d), and 15 g/(kg.d)], respectively, intragastrically for 28 days; piracetam (0.375 g/kg, intragastrically) was consecutively administered in the piracetam group; and normal saline was applied in the normal control and untreated groups. A Y-maze test was used for behavioral study to test the learning and memory abilities. A beta (1-42) and MMP-9 expressions in the hippocampus was determined immunohistochemically, and the results were analyzed by image acquisition and an analysis system. Results: Aggregated A beta (1-42) induced obvious learning and memory dysfunction, as well as up-regulation of A beta (1-42) expression in the hippocampus. Compared with those in the normal control group, the learning and memory abilities of rats in the untreated group significantly decreased (P<0.01), and the expression of A beta (1-42) was significantly increased (P<0.01). Twenty-eight days after different treatments, compared with those in the untreated group, the learning and memory abilities of AD model rats in the piracetam, low-dose, medium-dose and high-dose NEK groups were significantly improved (P<0.01 or P<0.05), and the expression of A beta (1-42) in the hippocampus decreased (P<0.01 or P<0.05), and MMP-9 increased (P<0.01 or P<0.05), especially in the high-dose NEK group. Conclusion: NEK might play a role of anti-dementia by increasing the expression of MMP-9 in the hippocampus of AD model rats, resulting in the reduction of the quantity of A beta (1-42) and improvement in learning and memory ability in AD model rats.

Abstract :

N-methyl-D-aspartate (NMDA) receptors (NMDA-Rs) have different modulatory effects on excitatory synaptic transmission depending on the receptor subtypes involved. The present study investigated the subunit composition of the presynaptic NMDA-Rs in layer II/III pyramidal neurons of the rat visual cortex. We recorded evoked a-amino-3-hydroxy-5-methyl-4-isoxazolepropionic acid (AMPA) receptor-mediated excitatory postsynaptic currents (eEPSCs) using whole-cell voltage clamp with the open-channel NMDA receptor (NMDA-R) blocker, (+)-5-Methyl-10,11-dihydro-5H-dibenzo(a,d)cyclohepten-5,10-imine hydrogen maleate (MK-801), in the recording pipette. We found that the paired-pulse ratio (PPR) by two successive stimuli with inter-pulse intervals of 50 ms was significantly increased by D-APV, a selective NMDA-R antagonist. Using a specific antagonist for NR2B-NMDA-Rs, (alpha R,beta S)-alpha-(4-hydroxyphenyl)-beta-methyl-4-(phenylmethyl)-1-piperidinepropanol hydrochloride (Ro 25-6981), instead of d-2-amino-5-phosphonovalerate (D-APV), we found that the PPR of eEPSCs was also significantly increased. Moreover, Zn(2+), an NR2A-NMDA-R antagonist, did not influence on the PPR. These results suggest that presynaptic NR2B-containing NMDA-Rs are located in layer II/III pyramidal neurons of the rat visual cortex, and that presynaptic NR2B-containing NMDA autoreceptors but not NR2A-containing NMDA autoreceptors mediate glutamate release in the rat visual cortex. Moreover, these findings may be clinically relevant to schizophrenia, where enhancing NMDA-R function is considered to be a promising strategy for treatment of the disease.
